POSITION PURPOSE: | The position serves as a support role to the MSW(s) and shall be instrumental in coordinating social supports and community resources for participants as well as participate in team discussion and explanation of the care plan document. This role may also support other roles as needed, including quality initiatives focused on improving services delivered to PACE participants or arranging for transitions of care for PACE participants as directed by the MSW &/or PACE Director.

POSITION PURPOSE: | Under the supervision of a licensed Occupational Therapist (pursuant to the laws of the state), the Certified Occupational Therapy Assistant (COTA) assists in patient evaluation, treatment planning and implementation. The Occupational Therapist (OT) evaluates patient’s cognitive, perceptual, & motor assets and liabilities for functional performance. POSITION PURPOSE | The Physical Therapist (PT)/Clinic Manager is responsible for: | - Day-to-day operations of the clinic | - Supervision of therapy and support staff | - Program development and provision of continuing education | - Providing goal-directed, functional activity-oriented, treatment to promote optimum independence in self-care, functional mobility, and daily life tasks | - Evaluating monthly financials to determine profit/loss, budget, cost efficiencies | - Communicating marketing strategies and needs for existing and new referral avenues | - Communicating with Director any problems and/or potential areas of concern that might affect revenue and/or staffing | - Evaluating the effectiveness of support staff and therapy staff on a clinical level and a revenue level | - Evaluating clinic equipment and staffing needs monthly | - Implementing strategies to ensure proper documentation by therapy and support staff
POSITION PURPOSE: | The Food Service Aide assists in the kitchen area prior, during, and after meals are served to participants. Assists in maintaining kitchen work areas, and keeping equipment, utensils and food plates clean and orderly.
