    POSITION PURPOSE:

    Responsible for the development and implementation of a program for continuous monitoring of patient care practices and outcomes in a consistent effort to maintain and improve the services and environment provided and assures compliance with CMS and state regulation.

    Qualifications

    POSITION REQUIREMENTS:

    License or Certification:  CPR Certification

     

    Total Education, Vocational Training, and Experience:  Licensed care giver preferred in a healthcare related field.  At least 1 year experience working with a frail, elder population.  Demonstrated expertise in the areas of performance activities and risk management.      

     

    Skills and Abilities:

    Ability to set priorities, possess good organizational and communication skills;

    Ability to type 40-60 wpm;

    Possess good judgment, tact and ability to work effectively under stressful conditions;

    Self reliance;

    Ability to take initiative and be self-motivated.

     

    PROTECTED HEALTH INFORMATION:

    Position has Limited Access to Protected Health Information of participants on caseload.

     

     

     

     

    Proficiency or Productivity Standards:

    Organize and prioritize tasks and projects

    Accurately complete job responsibilities within the specified time constraints to meet the needs of patients, regulatory standards and customer satisfaction
