    POSITION PURPOSE:

    Responsible for day-to-day oversite of PAC Team operations. Strategically leads the multidisciplinary PAC care team to improve patient throughput, appropriate level and quality of care assignments, and re-admission reduction from PAC providers. Interacts routinely with referral sources, internal and external multidisciplinary team members, care team members, as well as patient family members to ensure patient driven goals and outcomes are communicated, documented and reported to the appropriate entities.

    Qualifications

    POSITION REQUIREMENTS:

    License or Certification: 

    A Degree in Health Administration preferred not required.

     

    Total Education, Vocational Training, and Experience:

    At least 5 years health care experience preferred. Must possess knowledge of nursing, therapies, medical practices and procedures.

     

    Skills and Abilities:

    Knowledge and demonstrated capacity for leadership skills are required;

    Ability to make independent decisions;

    Must keep abreast of changes in industry;

    Must be able to interpret financial statements;

    Ability in promoting public relations and referral sources;

    Participation in health associations and educational programs indicative of professional growth is required.

     

    PROTECTED HEALTH INFORMATION:

    Position has Full Access to Protected Health Information of patients on caseload.
