    POSITION PURPOSE

    The SNF Coordinator manages coordinating SNF appropriate patients through the multidisciplinary PAC care team to improve patient throughput, appropriate level and quality of care assignments, and re-admission reduction from PAC providers. Interacts frequently with multidisciplinary team members, care team members, as well as patient family members to ensure patient driven goals and outcomes are communicated, documented and reported to the appropriate entities. The SNF Coordinator will guide the development and is responsible of the execution of the overall SNF appropriate PAC plan of care.

    Responsibilities

    Qualifications

    POSITION REQUIREMENTS

     

    Licenses or Certifications:  Licensed professional preferred but not required.

     

    Total Education, Vocational Training, and Experience:  2 years SNF experience preferred but not required.

     

    Skills and Abilities:

    Knowledge and demonstrated capacity for leadership skills are required;

    Ability to make independent decisions;

    Must keep abreast of changes in industry;

    Must be able to interpret financial statements;

    Ability in promoting public relations and referral sources;

    Participation in health associations and educational programs indicative of professional growth is required.

     

    PROTECTED HEALTH INFORMATION:

    Position has Full Access to Protected Health Information of patients on caseload.
