    POSITION PURPOSE:

    Evaluates and treats participants’ mobility and motor assets and liabilities for functional performance. Provides goal directed, activity oriented treatment to promote optimum independence in self-care, functional mobility and daily life tasks. Ensures continuation of performance through effective education of the participant, family or support system. Provides adaptive methods and equipment as appropriate to enhance performance. Participates in family / Participant care plan meetings, IDT and various committees as determined by Therapy Director.

    Qualifications

    POSITION REQUIREMENTS:

    License or Certification: CPR Certification, Licensed Physical Therapist in the State of Oklahoma

     

    Total Education, Vocational Training, and Experience:  At least 1 year experience working with a frail, elder population.

     

    Skills and Abilities:

    Ability to communicate with providers, team members, participants and families;

    Protect confidentiality in all areas as applicable;

    Knowledge of legal and ethical considerations in physical therapy practice;

    Ability to keep accurate records and write meaningful reports;

    Ability to execute therapeutic procedures and manipulate special equipment accurately and safely

     

    PROTECTED HEALTH INFORMATION:

    Position has Full Access to Protected Health Information of patients on caseload.
