POSITION PURPOSE:
To serve as the interdisciplinary team coordinator; facilitate the delivery of services to patients and their families; manage the financial resources of the patient by acting as a liaison to the payor; assume cost effectiveness of treatment provided by the team; facilitate and coordinate the discharge planning process; serve as the patient and family advocate; coordinate and direct effective communication regarding the patient’s care

POSITION REQUIREMENTS:
Licenses or Certifications: Current RN or LSW Licensure preferred, or other disciplines as deemed appropriate.
CPR Certification.
Total Education, Vocational Training, and Experience: Two to three year experience in medical rehabilitation
Skills and Abilities:
Working knowledge of regulations and parameters of third-party reimbursement;
Exhibits tact, diplomacy, and team-player attitude working toward customer and hospital-wide objectives;
Protects confidentiality in all areas as applicable;
Ability to dictate patient documentation;
Ability to demonstrate skill in administration, accountability, decision-making, and time management;
Ability to interact in a positive manner and communicate well with patients, families, co-workers, supervisor,
and other hospital employees;
Ability to coordinate the day-to-day function of the Department of Case Management.

Position has Full Access to Protected Health Information of patients on Rehabilitation Unit and in Outpatient.
