    POSITION PURPOSE

    Responsible for the psychosocial needs of the hospice patients and patient’s family. Provide mental and emotional support, monitor patient safety, and assist with adaptation to changes associated with end of life issues.  Provide education on community resources and make referrals as needed

    Qualifications

    POSITION REQUIREMENTS

    Licenses or Certifications:  N/A

    Total Education, Vocational Training, and Experience:  Bachelor’s degree required in Social Work, Master’s degree preferred from an approved school of Social Work.  1-2 years in health care field, hospice or LTCF experience preferred.

     

    PROTECTED HEALTH INFORMATION:

    Position has Full Access to Protected Health Information of patients on Rehabilitation Unit and in Outpatient.
