  • Overview

    POSITION PURPOSE:

    The position serves as a support role to the MSW(s) and shall be instrumental in coordinating social supports and community resources for participants as well as participate in team discussion and explanation of the care plan document. This role may also support other roles as needed, including quality initiatives focused on improving services delivered to PACE participants or arranging for transitions of care for PACE participants as directed by the MSW &/or PACE Director.

    Qualifications

     

    Total Education, Vocational Training, and Experience –Three (3) to five (5) years of related job experience preferred and at least one (1) year experience working with the frail or elderly population required.              

     

    Skills and Abilities:

    Exhibits tact, diplomacy, and team-player attitude working toward participant and facility-wide objectives;

    Protects confidentiality in all areas as applicable;

    Ability to appropriately document Participant interaction;

    Ability to demonstrate skill in administration, accountability, decision-making, and time management;

    Ability to interact in a positive manner and communicate well with participant, families, co-workers, supervisor, and other PACE employees;
