Home Health Aide/CNA Pace

POSITION PURPOSE:

Responsible for the provision of activities of daily living as identified by the Home Care Coordinator on the plan of care.

Licenses or Certifications:  Home Health Aide certification

Total Education, Vocational Training, and Experience: HS Diploma or equivalent with certification as a Nursing Assistant.  Certified to pass Home Health Aide Competency Test and Skilled Competency Checklist.  One year of Home Health Aide or LTCF experience. Minimum one year experience working with a frail, elderly population.

SKILLS AND ABILITIES:  Exhibits caring attitude toward patients and families.  Must be mature and able to cope with the demands of the job. Bilingual in Spanish or Vietnamese is beneficial.

PROTECTED HEALTH INFORMATION:

Position has Full Access to Protected Health Information of patients on caseload.

ENVIRONMENTAL CONDITIONS

Indoor climate-controlled environment.  Patient care environment with potential exposure to unpleasant odors, to blood and body fluids which may carry infection to infectious disease, and to chemical and electrical hazards.  Daily travel with exposure to outdoors elements.  Must have reliable transportation and maintain adequate automobile insurance coverage.
